Real-time monitoring of use patterns and trends of abuse can be employed to ensure state-of-the-art pharmacovigilance for products that are new to the market. New products with limited market share require near real-time data collected using sentinel, high-risk populations that are likely to seek out and try new or difficult-to-obtain products. Such data have been used productively to monitor newly marketed abuse deterrent prescription analgesic medications and to reassure regulators that any unanticipated outcomes will be detected rapidly. Post-marketing surveillance reports are customized to the particular product, API and formulation, so as to provide sufficient context for interpreting findings. While passive Internet monitoring yields important information on discussions that occur among recreational abusers who post online, we also use consented surveys to address specific questions about a product, comparators, abuse patterns and abuser preferences in a targeted and specific way. With the advent of policy changes and abuse deterrent technologies, surveys provide context that assists public health stakeholders in understanding the observed trends in misuse, abuse, tampering, and diversion.
